Summary
This study is testing a new nurse-led care program called COORDINATE, designed to help older adults who have two or more chronic health conditions and are leaving the hospital. The program provides personalized support, helps set health goals, and tracks symptoms over three months. Researchers want to see if this approach improves quality of life and reduces emergency room visits and hospital readmissions compared to standard care.
