Summary
This study tested a program that helps coordinate care for people with Alzheimer's disease and related dementias, along with their caregivers. Over 2,000 people on Medicare Advantage plans took part. The goal was to see if this program reduces emergency room visits and delays the need for nursing home care compared to usual care.
