Summary
This study tested whether having a clinical pharmacist review medications when patients are moved to the intensive care unit (ICU) reduces errors. The researchers compared prescriptions checked by pharmacists versus those checked by ICU doctors alone. They included 126 patients transferred from other hospital departments to the ICU at Alexandria Main University Hospital. The goal was to see if pharmacists could catch mistakes like wrong doses or missing drugs, making the transition safer.
