Summary

This study will compare standard postpartum care to a program called CAREPATH, which adds three telehealth visits with a community health worker. About 500 new mothers will take part to see if the extra support helps them complete their postpartum checkup, catch complications early, and avoid hospital readmissions or emergency visits. The goal is to improve health after childbirth.
