Summary
This study tested a program called CONTINUUM for people with advanced cancer leaving the hospital. Within three days of going home, patients had a video visit with a specialized nurse to support their transition. The goal was to see if this extra support helps patients feel more confident managing their health. The study involved 286 adults from one hospital in Massachusetts.
