New study aims to make Hospital-to-Home transition safer for cancer patients

Summary
This study tests whether a nurse-led program using online questionnaires and phone calls can help cancer patients transition safely from hospital to home. About 50 adults with cancer who own a smartphone or tablet will participate. The goal is to see if this follow-up approach is practical and improves patient satisfaction and quality of life.
