Summary
This study is testing two different team approaches to help people control high blood pressure. One approach uses remote monitoring with medical professionals, while the other adds community health workers who help with both medical needs and life challenges like housing or transportation. Researchers are comparing these approaches to standard blood pressure screening to see which works best for long-term health.
