Summary
This study looks at whether having a hospital pharmacist share information with a patient's regular community pharmacist can reduce medication mistakes after a hospital stay. It involves 150 adults who take 7 or more drugs and are being discharged home. The hospital pharmacist works with the community pharmacy, not directly with the patient, to improve the safety of discharge prescriptions.
