New home care program aims to keep seniors out of hospitals and nursing homes

Summary
This study tests a program that combines home care with technology to help frail older adults live at home as long as possible. About 1,000 people aged 70 and older with complex health and social needs will take part. The program uses a team approach and digital tools to reduce hospital stays and improve quality of life.
