Summary
This study tested a new scoring tool designed to help emergency room staff identify patients at high risk for serious medication problems that could send them back to the hospital. The tool flagged patients so a pharmacist could review all their medications before they went home. Researchers wanted to see if this extra check could reduce the number of patients who return to the ER or get hospitalized due to medication issues within 30 days.
