Summary
This study is testing a new remote care program designed to support elderly nursing home residents right after they leave the hospital. It connects nursing home staff and family doctors with geriatric specialists via telemedicine. The goal is to see if this extra support can reduce the need for residents to go back to the hospital and improve the management of their overall health conditions.
