Summary
This study is testing a personalized support program for families who provide home care for a loved one with a serious, advanced illness. It aims to see if working with a nurse to create a personal care plan can improve the family's caregiving experience, reduce their stress, and better prepare them for their role. Researchers will compare families who receive this extra support to those who receive standard care alone.
