New study aims to prevent breakdowns for disabled individuals and their caregivers

Summary
This study looks at whether a short stay in a special transitional care unit can help people with disabilities and their main caregivers avoid crisis situations. The goal is to see if setting and achieving personal goals during the stay improves support and reduces the need for emergency care. About 100 participants (patients and their caregivers) will take part to gather knowledge on how to better prevent exhaustion and breakdowns at home.
