Summary
This study aims to find the best way to help adults with multiple long-term health conditions get the social support they need. Researchers will compare two approaches: one with more personal phone support from a health guide, and another with less intensive digital outreach. The goal is to see which method better helps people access services, meet their social needs, and close gaps in their medical care.
