Summary

This study tests a structured, personalized exercise program for 150 people with heart failure with preserved ejection fraction (HFpEF), a condition where the heart pumps normally but doesn't fill properly. Participants will follow a 12-week smart exercise plan with remote monitoring to see if it improves their exercise capacity, heart function, and quality of life. The goal is to find a safe, effective way to ease symptoms like fatigue and shortness of breath.
