Summary

This study looked at whether having a clinical pharmacist review medications daily, with help from a mobile app, could reduce errors in hospitalized children aged 2 to 15. About 2000 kids took part, some getting usual care and others getting extra pharmacist checks. The goal was to see if this approach catches and prevents medication problems, making hospital stays safer.
