Summary
This study tests whether personalized remote monitoring at home helps patients recently hospitalized for heart failure. It compares standard care to a program where nurses use technology to check on patients remotely, aiming to catch problems early and prevent return trips to the hospital. The goal is to see if this approach reduces readmissions and improves patients' quality of life and self-care.
