Summary

This study tested a new digital tool designed to help doctors and pharmacists identify and safely reduce medications that can increase the risk of falls, confusion, and memory problems in older hospital patients. The tool automatically flagged patients with a high burden of these 'anticholinergic' drugs, prompting their care team to review and adjust prescriptions. Researchers worked with 21 patients and hospital staff to see if this collaborative approach was practical and acceptable for improving medication safety.
