Summary
This study tests whether a detailed medication review for hospitalized patients aged 65 and older who take at least seven drugs can lower the chance of returning to the hospital or visiting the ER within 30 days. The program involves doctors and pharmacists working together across hospital and community settings. About 800 participants will be split into two groups to compare outcomes.
