Summary

This study tested a new support program for women with high blood pressure after giving birth. The program included using a phone app to report blood pressure readings, guidance from a care navigator, and help connecting to a primary care doctor. Researchers wanted to see if this approach improved blood pressure control and reduced health risks, especially for Black mothers who face higher risks.
