Summary

This study tests whether having a clinical pharmacist work with hospital patients from admission to discharge can reduce unplanned hospital visits and improve medication safety. About 610 adults with multiple health conditions will be randomly assigned to either standard care or care that includes a pharmacist who reviews medications, joins medical rounds, and provides drug education. Researchers will track hospital visits, side effects, and patient satisfaction for six months after discharge.
