Summary
This study tested a one-page Patient-Oriented Discharge Summary (PODS) for older adults leaving the hospital. The goal was to see if it improves the quality of the transition from hospital to home and reduces problems after discharge. 111 patients who could speak French and were going home after a hospital stay of more than 48 hours took part. The study measured how well patients understood their care plan and how prepared they felt.
