Summary
This small pilot study is testing whether having a pharmacist help patients with their medications when they leave the emergency room can prevent mistakes and reduce return visits. The program includes reviewing prescriptions, updating medication lists, and follow-up calls. The goal is to see if this approach works well enough to test in a larger future study.
