Summary
This study tests whether a pharmacist-led program can reduce medication errors when patients leave the emergency department. About 82 adults at high risk for drug problems will either get usual care or extra pharmacist help, including medication review and follow-up. The goal is to see if this approach is practical and acceptable before planning a larger trial.
