Can a hospital pharmacist at your bedside keep you from coming back?

Summary
This study tests whether having a clinical pharmacist closely involved during a patient's entire hospital stay improves care and reduces problems after going home. About 610 adult patients admitted to the hospital who take multiple medications or have certain health conditions will be randomly assigned to receive either this special pharmacist service or standard hospital care. The main goal is to see if the pharmacist's help leads to fewer emergency visits or re-hospitalizations in the six months after discharge.
