Summary

This study looks at whether customizing digital health messages can help people with uncontrolled high blood pressure keep doctor visits, take medications, and check their blood pressure at home. Researchers will enroll 12,000 adults from Cone Health who have high readings. The goal is to see if personalizing outreach leads to fairer and better health outcomes.
