Heart failure network trial aims to improve recovery for 1000 patients

Summary

This study looks at whether a special heart failure care network can help people who were recently in the hospital for heart failure. About 1000 adults with moderate to severe heart failure will take part. Half will get the network care, and half will get usual care. The main goal is to see if the network improves a key heart health marker over 6 months.
