New program aims to ease burden on family caregivers of chronically ill patients

Summary
This study tests a person-centred care program to help families who care for a loved one with an advanced chronic illness at home. About 50 families will either receive the program plus usual care or usual care alone. The program includes an in-person meeting with a nurse to create a personalized care plan, followed by monthly check-ins for 3 months. The goal is to see if this support improves caregivers' experience, preparedness, and emotional wellbeing.
