New online tool aims to boost Nurse-Patient teamwork in chronic care

Summary
This study is creating and testing an online educational program that teaches nurses and people with chronic diseases how to make medical decisions together. About 288 participants (nurses and patients) will try the 4-week program. The goal is to see if it improves teamwork, communication, and confidence in making health choices.
