Summary
This study aims to help more nursing homes adopt a proven care model that reduces unplanned trips to the hospital for residents. It focuses on finding a cost-effective way to spread this model, which uses specially trained nurses and better teamwork to manage care. The research will compare two different support plans across 40 nursing homes to see which works best for long-term use.
