Summary
This study tests whether a nurse-led program called NUR-FRAIL can help older adults (65+) with chronic conditions manage frailty and improve self-care. Two hundred participants will either receive the program or usual care. Researchers will measure changes in self-care, quality of life, and hospital use over six to twelve months.
