Summary
This study is testing whether a special 3-month program led by nurses can help older adults with chronic conditions prevent or manage frailty. The program provides personalized education, lifestyle advice, and support to help people better care for themselves. Researchers will compare 200 participants who get this program to those receiving usual care to see if it improves self-care, quality of life, and reduces emergency room visits and hospital stays over a year.
