Summary

This study looks at whether a nurse-led program can help adults over 65 with long-term health conditions better manage their own care and reduce trips to the hospital or emergency room. About 100 participants will get personalized education and at least three follow-up sessions over six months. Researchers will compare their health outcomes before and after the program to see if it works.
