Summary
This study looks at whether a nurse reaching out after a clinic appointment (by phone or patient portal message) can help adults avoid unnecessary emergency room visits. About 13,000 patients aged 30 and older with certain health conditions will be randomly assigned to get the nurse follow-up or usual care. The main goal is to see if the nurse contact reduces ER visits within a week.
