Summary
This pilot study is testing whether a structured, team-based care plan is better than usual care for managing diabetic kidney disease. It will involve 100 adults in Ontario with type 2 diabetes and early signs of kidney problems. The goal is to see if a proactive approach to starting and monitoring key medications can better control the disease and reduce heart risks.
