Summary
This study is testing a website called 'My Voice' designed to help people with serious kidney disease who are on dialysis. The website helps patients learn about their condition, think about their future care preferences, and guides them on how to talk about these wishes with their doctors and family. Researchers want to see if using this website leads to better, more meaningful conversations about future care compared to the usual practices at dialysis centers.
