Summary
This study tests a website called 'My Voice' that helps kidney failure patients on dialysis plan their future medical care. The website teaches patients about their illness, helps them decide what kind of care they want, and coaches them on how to talk to doctors and family. About 100 adults in Singapore will use the website to see if it improves the quality of these important conversations compared to usual care.
