Summary
This study is testing a new questionnaire designed to help doctors spot a wide range of side effects early in people who have finished curative treatment for breast or prostate cancer. The goal is to see if using this questionnaire leads to better quality of life by helping doctors intervene sooner. The study will compare the quality of life of survivors who use the questionnaire with those who receive standard care.
