Summary

This study tests whether a smartphone app, used with a pharmacist's help, can improve medication tracking, blood pressure, and blood sugar control in kidney transplant patients. The goal is to help African American patients have better long-term outcomes after transplant. About 190 adults who are at least 2 years post-transplant will participate.
