New program aims to stop dangerous drug Mix-Ups for elderly

Summary

This study is testing whether having a team of medication experts review prescriptions for elderly nursing home residents makes their drug regimens safer and more appropriate. About 219 residents will have their medications reviewed by clinical pharmacologists, who will then provide feedback and education to the nursing home staff. The goal is to see if this process reduces harmful drug combinations, unnecessary medications, and related health problems like falls and hospital visits.
