Summary

This study tests whether a team of medication experts can improve the safety of prescriptions for elderly people living in nursing homes. Researchers will review each resident's medications, give feedback to doctors and nurses, and provide training on common prescribing issues. The goal is to reduce inappropriate drug use, falls, and hospital visits. About 219 residents aged 65 and older will take part across seven nursing homes in Southern Switzerland.
