Summary
This study tested whether carefully reviewing and correcting a patient's medication list before leaving the hospital could help people with chronic kidney disease. Researchers compared 323 patients who received this extra medication check to a group that got usual hospital care. The goal was to see if this process reduced medication errors and prevented patients from being readmitted to the hospital within 90 days.
