Summary
This study tested whether a careful review of a patient's medications when they leave the hospital helps people with chronic kidney disease. Researchers compared a group that received this extra medication check to a group that got standard hospital care. The main goal was to see if the review reduced mistakes in medication lists and lowered the chance of patients being readmitted to the hospital within 90 days.
