Summary
This study looks at how to expand a successful nurse-led care model called INTERCARE to more nursing homes in Switzerland. The model aims to reduce unplanned hospital visits for residents. Researchers will work with 40 nursing homes to see how well they can adopt the program and what helps or hinders its use. The goal is to find a cost-effective way to improve care for older adults.
