Summary
This study tests a telehealth program for people aged 65 and older who have survived a stay in the intensive care unit (ICU). Participants get a recovery navigator who helps coordinate care after hospital discharge, including setting recovery goals and staying in touch with their doctor. The goal is to improve recovery and reduce preventable hospital readmissions.
