Summary

This study looks at whether older adults (65+) with high blood pressure and other health problems can better control their blood pressure by measuring it at home. Some participants will get extra support from a clinical pharmacist. The goal is to see if this approach lowers blood pressure and reduces the burden of managing multiple health conditions.
