New study aims to help transplant survivors talk about their care wishes

Summary

This study looks at whether a patient navigator or a website called PACT can help blood cancer survivors who had a stem cell transplant plan their future medical care. About 40 adults who are 1 to 5 years post-transplant will take part. The goal is to see if these tools help people have important conversations about the care they want.
