This study is for people over 65 who have survived a serious illness in the ICU. It tests whether giving patients and their doctors personalized information after hospital discharge helps them make better care decisions, improve recovery, and avoid preventable hospital readmissions. About 300 participants will be followed for 90 days to see if this approach reduces death, disability, and repeat hospital visits.
