New approach aims to cut medication errors when seniors leave the hospital

This study tested a program to help older adults (65+) better understand their medications when leaving the hospital. The program included an information package, better discharge paperwork, improved communication with doctors, and a follow-up call. The goal was to make patients feel more involved and reduce medication mistakes after they go home.
