Summary
This study aims to create and test a care bundle—a set of simple, evidence-based steps—to help heart transplant patients take their immunosuppressive (anti-rejection) medicine as prescribed. Taking this medicine correctly is crucial to prevent the body from rejecting the new organ. The study will involve 100 adult heart transplant patients and will measure how well the care bundle works over six months.
